What causes $s->protocol() to return the string "INCLUDED"?

SSI includes / mod_include

Take that back, I am wrong. It's set by the subrequest mechanism, so it
will be set for internal sub-requests.

An SSI virtual include:

<!--#include virtual="/foo/bar" -->

Is just one case, anything that triggers a subrequest will do it too.
