What you're after is a PerlAccessHandler
http://perl.apache.org/docs/2.0/user/handlers/http.html#PerlAccessHandler

Depending on how complex your rules are, you may be able to get away
with mod_access in apache:
http://httpd.apache.org/docs/2.0/mod/mod_access.html
