There are no references to the folder state image files in my Apache logs
because those files are not even being requested.  I do not even have the
following lines in the JavaScript header of pages for my installation of
the standard catalog which are certainly present in the official version.

var product_tree_toggle_anchor_closed = '<img src="/demo4/images/fc.gif"
width="13" height="13" border="0" align=absbottom>';
var product_tree_toggle_anchor_open = '<img src="/demo4/images/fo.gif"
width="13" height="13" border="0" align=absbottom>';

Perhaps incompletely set configuration files are responsible as suggested
by Kevin Walsh.
