[ic] pulldown State/Provence dissappear

Hi,

I changed the field-order in my pages/member/account.html. Registering the
shipping address you first get the country-field (pull down) and then the
State/Procence - field.

Problem: when I change the field/input order (country first) the standard
pulldown option for my State/Provence dissappear - I get a blank text
field instead of the pull down.

I have no idea what I am doing wrong.
