[ic] Menus and category list in 5.4.1

I had gotten used to the area/cat tables, but the new 'tree' table is a 
better design.  But the one time I tried to use 5.4 I I could not figure 
out how to generate the 'tree' table.   I must have missed something, 
but I searched the admin UI and the documentation,
