You probably didn't run the "mysql_fix_privilege_tables", as instructed
to in the MySQL 4.0 -> 4.1 upgrade guide.  Backup your database and
run it now.  That should fix it.

The servers I have that run MySQL were all upgraded to 4.1.20 a long
time ago.  4.1.20 is now the default version supplied with Gentoo
GNU/Linux.
