[ic] problems with "Standard"

I've set up a store with "Standard" and there are some problems:

1. on checkout I always get this error:
* (mv_same_billing): No format check routine for 'always_pass'
*Where/how do I fix this?  I can't seem to find information on that.

2. It seems I can not remove anything from the cart, whenever I click
"remove" the page reloads with the same information.  Is this a javascript
error in the default installation?
