I have the same problem, if I don't run in RPC, under extremely high
credit card transaction concurrency (one CC charge every 5 seconds) we
get duplicate charges.

Me too, but I have never associated it with a particular traffic mode. We run in RPC mode and get a few double charges each week. I believe this tends to happen when our PSP is responding slowly to payment authorisation requests, or perhaps when our server is under load. Consequently, I have assumed that customers click the back button on the browser just before the "Thank you for your order" page appears and then just assume that their order wasn't processed and so place a second order.

However, I also have strong suspicions that it relates to the Net::SSLeay problems that Mike Heins refers to (see my previous post in this thread), whereby customers are receiving an error message when trying to place their order, even though the PSP has in fact authorised and processed their payment. Indeed, we have had several customers tell us that they did indeed receive an error message the first time they tried to place their order, hence the reason they placed it again...
