I don't know what you are trying to accomplish exactly, so I might miss the point. We usually do such things with a cron job. One of our applications stores all information regarding hosting accounts in a database, including the databases that need to be created. The cron job checks if it needs to create a database and if so it'll create it and sets a flag that it has been created. This works really well for us. But as said maybe you've got different needs.
