[ic] Internal UPS Lookup Problem

I'm having difficulty getting an internal UPS lookup to work.

Everything works okay if I use one of the standard tables (Ground, 
2ndDayAir, etc), but if I use my own table no value is calculated.   
The only difference between a working config and one that doesn't is 
the name of the table.  The table is configured, shows up in the admin 
interface, and can be edited from the table editor.

I've searched the archives and haven't found anything that would show a 
solution.

Any ideas?
