[cgiapp] CGI::App Generator

I put together a first take on a new generator program, 
design/document-wise:

Might have made better sense to use a wiki, but I wanted to whirl on it 
now.  I'll make sure it's on our new wiki whenever that's up/ready.

Between now & then, feel free to use this list or my email.  As I say in 
the document, this is to help jumpstart cgiapp projects w/ common best 
practices ... but as of right now, it's just my own perspective on "best 
practices", so I hope to get some feedback.

PS: I unveiled this on the #cgiapp channel and folks like 
HTTP::Server::Simple vs. Apache::Test and Module::Build vs. 
ExtUtils::MakeMaker... so feel free to chime in on that, too.
